A Person-Centered Approach to Care Coordination
The Atlanta Regional Collaborative for Health Improvement (ARCHI), a collaborative of more than 110 organizational partners, is a driving movement that unites community perspective with institutional action to accelerate health improvement. During this session, Aisha Williams, Sharon Glass, and Kimberlee Scott, shared best practices on building partner networks, the value of interoperability and data sharing among health systems and community partners, and ARCHI’s Community Health Worker (CHW) model directly from one of their network CHWs and a community member with lived experience.
